We have opportunities for Carer Assessment & Support Officers to join n-compass across Lancashire, Bury, Staffordshire and Rochdale on a Casual basis with flexible hours to suit your circumstances.
The role of a Carer Assessment & Support Officer provides carers with information and support to reduce the impact of their caring role by:
- Contacting newly referred Carers to welcome and induct them to our service
- Undertaking Carers’ Assessments in their home, the community or by telephone, taking a 'whole family' approach to the assessment
- Considering whether or not advocacy support maybe required
- Assessing eligibility under the Care Act 2014 using the National Eligibility Criteria
- Developing a care and support plan, proportionate to the assessed needs.
- Utilising the Local Authority Resource Allocation System (RAS) determine a Carers’ payment and Commission Carers’ payments, explaining the payment process to the Carer.
- Undertaking an 8 week review
- Ensuring that Carers who are ‘not eligible’ in accordance with the Care and Support (Eligibility Criteria) Regulations 2014 are provided with advice, information, signposting and referral on to meet current needs and prevent/delay future needs
- Ensuring that Carers’ Assessments and Support Plans are complaint with the Carers’ Assessment and Support Planning Quality Assurance Framework
- Providing up to date information regarding carers rights and the services and resources available to support Carers
- Ensuring support offered and information provided to Carers is appropriate for their specific needs including but not limited to mental health conditions, learning disabilities and dementia.
- Maintaining accurate case recording using both Local Authority Case Management Systems and internal recording systems in line with business processes
- Supporting the delivery of carer forums and community-based Peer Support sessions
- To provide a quality standard of report to support service reporting
- Achieving individual performance management indicators as set by the Team Leader
- Promoting and safeguarding the welfare of children and adults
- Attending training and meetings as required
