Assessment & Quality Coordinator

We are looking for an experienced and committed Community Care Professional to lead our Client Assessments, Reviews & support our managers to oversee the quality of the care we deliver.
This role will be primarily office based with some frontline care to support our clients when needed and provide on the job support to our staff. This is a new and exciting role ideal for anyone hoping to make a positive impact on the lives of the individuals we support whilst progress their career in Adult Social Care.
**As a Assessment & Quality Coordinator, you will carry out a broad range of duties to support our clients in their homes which includes**:
- Undertake initial assessments for new clients
- Write and Review Care Plans and Assessments for existing and new clients to ensure we are supporting our clients to achieve positive outcomes in person-centred ways
- Undertake Care Plan Reviews with Clients
- Overseeing that all care plans are understood and followed through by our carers and all paperwork is kept up to date.
- Making sure the client and their family are happy with the care & support provided
- Supporting our Care Team to develop their skills by demonstrating best practice and providing on the job training.
- Carrying out spot checks on members of our Care Team
- Contribute to the rota of On-Call support providing assistance to our care team in emergency situations
- As required support with frontline Care Delivery working one weekend on one weekend off

**Job Types**: Full-time, Part-time, Permanent
Part-time hours: 40 per week
**Salary**: £25,000.00-£27,000.00 per year
