We have recently extended our funding to expand our activities and discharge home support provision to provide **stimulating activities to** **older people in a hospital setting** **and to provide further support to patients on their return back home**. This is a real opportunity to make a difference and we are looking for experienced, creative and passionate individuals to join our team.

Hospital Discharge Activity Coordinators
- 25 hours to be worked across 4 days Monday to Friday between the hours of 9.30am and 3pm

£14,872 actual plus pension and 30 days annual leave plus public holidays (pro rata)

Do you have experience of supporting older people living with long term health conditions including memory loss and dementia? Would you enjoy getting to know individual customers and their life stories and relish the opportunity to design and deliver individual and group activities that maximise wellbeing during their stay in Hospital. Able to engage in stimulating conversations and make connections to discuss what support they may need to resume living independently on their return home.

These roles will provide an agreed programme of short term support in their own homes to older people on discharge from hospital ,carrying out a wide range of practical tasks to a high standard within the required timescale.

These tasks may include (but are not limited to):

- Supporting customers with reading and dealing with correspondence, collecting pensions, paying bills, setting up direct debits
- Enable customers to shop, cook and eat - this may include assisting with preparation of a shopping list, accompanying customers to the shops and/or enabling customer to shop ‘on-line’
- Enable customers to set up and access appointments

**Car drivers are essential to this role**

Cognitive Stimulation Therapy and Dementia Stars training will be provided.

Pay: £11.44 per hour

Expected hours: 25 per week
