An exciting opportunity has arisen for an experienced Children/ Adult registered nurse to join as our Neonatal team as Discharge Coordinator
The unit accept infants from 27 weeks of gestation. The unit has 4 intensive care, 5 high dependency and 17 special care cots, making a total of 26 cots.

We are looking to recruit a motivated member of staff willing to provide energy and enthusiasm to drive forward our Neonatal Discharge Coordination and planning services within the Neonatal Unit and Neonatal Directorate. We are seeking an individual with experience of working within the NHS, with clinical teams and with an understanding of leading and providing comprehensive support for infants and families on the Neonatal unit
You will be expected to be a self-starter with an ability to challenge in a constructive manner. This is an exciting opportunity providing broad exposure across a variety of settings.
- Work in partnership with the MDT to actively determine the best place for continuing care of each baby, whether this is staying on the unit, transfer to their local hospital, home or other ward. This will include working closely with the Paediatric community services, palliative care sister, community midwives and the MDT to actively and safely reduce the length of stay of the babies on neonatal unit.
- Provide formal and informal teaching sessions to staff across the Division to facilitate change in clinical practice ensuring babies and their families receive excellent care that is evidenced based.
- Develop and improve processes and structures as potentially amalgamating existing processes across maternity Neonatal and Paediatric services ensuring smooth transition across services. It will be essential therefore for you to have excellent communication, negotiation and planning skills to influence and also drive forward the requirements.
