We are currently offering an exciting opportunity for a Transfer Facilitator to join the Patient Flow Team at Antelope House in Southampton.

We are looking for a skilled and dedicated individual who has a passion for working with individuals who are experiencing mental health difficulties and their families to deliver high quality care and support.

As a Transfer Facilitator, you will work with agencies involved in the care pathways of clients and liaise with care teams to coordinate access to relevant interventions, support and self-management for clients alongside the Crisis Team (CRHTT), Inpatient wards, Community Mental Health Teams (CMHT) as well as other health & social care providers.

You will support the planning, coordination, and transfer of service users across care pathways, liaising with Southern Health Foundation Trust units and other NHS or private providers as required. It is important that you enjoy working in a busy environment, managing multiple priorities, planned & unexpected, being able to work to a level of flexibility, accountability, and autonomy under pressure as part of a team.

Working alongside the Flow Team Leader you will also provide support, guidance and supervision to a small team of Band 4 Care Navigators who undertake practical support to inpatient service users to overcome barriers to discharge such as accommodation and finances.
