**Project Officer: Virtual Transfer of Care (VToC) - Avoiding Hospital Admissions**

Venn Group is currently working with a large NHS company who are looking for a Project Officer. Below is a brief synopsis on the role;
**Duration: Start date - ASAP - End of March**

**Location: Southampton - Potentially flexible with hybrid working**

**Hours: 37.5**

**Day to day duties include**:

- Supporting the roll out of VToC across Southampton Social Care providers in particular care and nursing homes
- Providing digital support - might need to go to care homes around Southampton and physically help the nursing staff use the iPad with the assessor on the other end from UHS
- Collecting data and liaising with stakeholders
- Working under the direction of a band 7 who be leading the project within an acute hospital setting to implement a digital transformation project within social care. The VToC project will support with winter pressures and support admission avoidance
- Help management in care home etc on how to use certain programmes and find their way around the system
- Robust evaluation and data collection will be a key component to measuring the success of the VToC project and the officer will support the project manager with this
- Responsible for helping to set up a network across social care relevant to the project in Southampton to facilitate rich communication and share best practice.
- Providing tailored training where appropriateto staff across all pre agreed levels/arenas to enable the VToC project to succeed
- Assist the wider teams in a range of support and engagement tasks as required including working on the Wards and in the Community

**Essential Skills for the role**:

- Good understanding of IT products and be digitally savvy with iPads and how to troubleshoot
- A strong understanding of health or social care
- Be confident in supportingIT products to individuals and groups who may not have specialist knowledge - knowledge of care home settings would be an advantage
- Accuracy and attention to detail
- Confident, positive personality with excellent interpersonal skills and a professional manner
