**Job summary**

The role of the Care Co-ordinator is to proactively:

- Identify patients who are deemed as high users and support practices with non-medical interventions and referrals to external partners.
- To support the multi-disciplinary team (MDT) meetings and the ongoing patient case management.
- To liaise with the additional roles staff in the PCN to meet unmet patient needs.
- Support patients to retain their independence.
- Engage with patients living in care homes.
- Support practices with housebound patients.
- Act as a link between external agencies and General Practice.

**Main duties of the job**
- Proactively identify and work with a cohort of patients to support their personalised care requirements.
- Aid with identifying and supporting health inequalities and health promotion work.
- Support the development of a single personalised care and support plan with patients and family members to bring together their identified care needs.
- Ensure that patients have good quality information to help them make informed choices about their care.
- Assist patients to access self-management education courses, peer support or interventions that support them in their health and well-being.
- Navigate and signpost patients and their carers appropriately, working alongside social prescribing link workers, health and wellbeing coaches and other primary care roles.
- Develop and maintain effective and productive relationships with practice staff and other teams, referring and liaising with them in the best interest of the patients care.
- Use practice IT systems to accurately record patient care and manage their health.
- Assist and support in vaccination programs as required.
- Research and actively seek new opportunities to develop the role.
- Be the person on the phone for some of our most vulnerable patients needing support.
- Support carers by identifying unpaid carers and directing them to access services as appropriate.
- Identify when urgent action or a step up in care is required and promptly alert the relevant professionals involved, highlighting any safety concerns.

**Qualifications**:
Essential
- Level 2 or GCSE pass in English, functional skills accepted.

Desirable
- Care Certificate

Experience
Essential
- Experience of working in healthcare.
- Experience in direct patient care.

Desirable
- Experience in using SystmOne.
- Experience of working in Primary Care.
- Experience in using other practice-based software.
- Trained in Care Navigation or similar.
- Experience in working with the voluntary sector.

Other

- Can work on own initiative and prioritise workload.
- Excellent time management.
- Ability to handle multiple tasks concurrently.
- Ability to organise oneself and others effectively.
- Good verbal and written communications skills.
