Northern Healthcare provides mental health recovery services across many regions within the UK. Our services consist of Supported Living and community outreach services which provide our residents with access to 24 hour support from experienced clinical and support staff.

Currently our services are based across the Northwest, Cumbria, Yorkshire and Midlands supporting over 150 adults across the UK in all aspects of their daily lives and mental health recovery. We have developed a unique clinically led, recovery-focused support model which provides individuals with dedicated 24-hour support in high-quality accommodation.

We are looking to recruit an enthusiastic and motivated individual to join our team as an Assessment Practitioner.

The Assessment Practitioner will be responsive and promote a dynamic and innovative way of responding to referrals, offering face to face assessments in a timely manner while maintaining high standards of documentation incorporating robust assessment outcome and formulation with robust risk management plans.

You must be enthusiastic, person-centered, and able to provide a flexible and creative approach to care delivery. We are looking for a highly motivated team player who can work under pressure, someone who is interested in quality improvement and have ideas about how we can improve the experience of people accessing support in a variety of situations.

We are keen to hear from professionals with a range of experience in either Mental Health or Learning & Disability and Autism. Applicants should have excellent written and verbal communication skills and be able to interact at all levels. Attention to detail and proven ability to work to tight deadlines are essential for this role, as well as a high level of accuracy, data management skills and proven IT skills particularly in Electronic Records. Depending on experience within the mental health field, the following core professional roles will be considered for these posts - Registered Nurse Mental Health, Registered Nurse Learning Disabilities, Social Worker, Occupational Therapist, Psychological Wellbeing Practitioner.

You will be based at Northern Healthcare head office in Eccles and you will be expected to cover nearby sites.

The role is offered on a full time basis - and can support flexible working arrangements appropriate to service need

Key Responsibilities

The post holder will be working as an autonomous practitioner and will play and an active role in the triaging processing and assessment of referrals made to Northern Healthcare.
- To work with the referrals teams to gather and process complex information from a variety of disciplines and agencies regarding initial referrals to Northern Healthcare.
- To develop care and support plans that meet the needs of the individual and their family/carers as appropriate and that are person centred, focused on strengths, and are outcome based.
- To take responsibility in ensuring that risk assessments and management plans are carried out in an active and positive manner.
- To ensure that assessment methods and intervention models and processes are evidenced based and appropriate to their circumstances and within their capability.
- To ensure that relationships and own behaviours are managed in line with organisational values
- To ensure all communications are undertaken in a sensitive, confidential, and professional manner.
- To ensure that clinical best practice, local organisational policies and procedures, national standards and registration are always adhered to.
- To always act within the relevant professional code of conduct
- To support the on-call process for out of hours support for NHC
- To participate in NHC governance meetings
