**Main duties and responsibilities**: Keep up to date with policies, guidelines, procedures and practices. Participate in team meetings and other meetings as required. Represent Seacroft PCN in a knowledgeable and professional manner at all times. Maintain appropriate professional boundaries at all times.
To be able to maintain an effective efficient level of working alongside an active case load and manging time appropriately. Identify own training and development needs in conjunction with your line manager and participate in training opportunities as directed. Support all victims and survivors aged 16 and above with current or historic domestic abuse. Support all those effected by domestic abuse in the household, having a keeping children safe approach.
Provide advocacy, emotional and practical support and information to victims, survivors including but not limited to legal options, housing, health and finance. To attend with victim survivor to court, CPS, VRI and other professional appointments where necessary. Work in partnership with key safeguarding agencies to address the safety needs of high risk victims. Ensure all victims and survivors safety plans are regularly updated and relevant, consistent updates and actions are shared with other agencies such as MARAC and designated safeguarding leads, with consent, unless for relevant safeguarding needs consent does not need to be obtained.
Manage a case load ensuring each victim survivor receives the appropriate service, individual to their needs. Support empowering the victim survivor, to regain personal control of their choices, and ability to continually support themselves when DASSP support ends. Support the victim survivor to recognise domestic abuse which relates to their current situation and possibly historic relationships. Work closely with external agencies to meet the victim survivor support needs where those needs are specialist complex eg Mental Health, Substance Misuse, BME etc.
Provide an easily accessible source of expertise and support to GPs and clinical staff in the management of individual cases as required. Accept referrals from directly through Remote booking slots, GP Confed and direct bookings into S1 PCN rota and encourage routine enquiry. Develop and maintain excellent working relationships with GPs and all practice staff. Work flexibly across the practices identified Provide DVA awareness inputs to GPs and practice staff Such as case studies at team meetings and support practices to obtain the Safer stronger communities Leeds DVA Quality Mark.
To record all notes on S1 through the PCN Unit, and to ensure sensitive notes are marked hidden from online view. The above duties are indicative of the requirements of the post at the time of recruitment. You will be expected to undertake other duties as may be reasonably required commensurate with the post, at the initial place of work or at other locations from which Seacroft PCN operates. These policies detail the employees legal obligations and include references to current legislation.

Risk management You will be responsible for adopting the risk management culture and ensuring that you identify and assess all risks to your systems, processes and environment and report such risks for inclusion within the South & East Leeds General Practice Group risk register. You will also attend mandatory and statutory training, report all incidents/accidents, including near misses, and report unsafe occurrences as laid down within the PCN Policy.
