PCN Clinical Pharmacist – NHS Join to apply for the PCN Clinical Pharmacist role at NHS. Job Summary We have an exciting opportunity in our Primary Care Network for a clinical pharmacist to join our team. The role will support the development of medicines optimisation services within our PCN. Morley has a well‑established PCN made up of 6 GP practices covering 63,000 patients. The role may be working across any site within the PCN. Responsibilities Provide clinical medicines management, structured medication reviews, and management of long‑term conditions. Conduct face‑to‑face and telephone medication reviews for patients with polypharmacy needs, especially older people. Offer quality improvement, clinical audit, and support management of aspects of the Quality and Outcomes Framework. Make recommendations to GPs and nurses on prescribing, monitoring, and medicine optimisation. Risk‑stratify high‑risk patients through computer searches and reduce prescribing of medicines associated with unplanned admissions. Reconcile medicines following discharge from hospitals, intermediate care, and care homes. Provide medicine information to GP and MDT staff and answer enquiries. Collaborate with the Lead PCN Clinical Pharmacist, Pharmacy Technician, prescription teams, and dispensary teams. Manage repeat prescribing policy and re‑authorisation process. Analyse, interpret, and present medicines data to support decision making. Implement changes arising from MHRA alerts, product withdrawals, local and national guidance, and formulary recommendations. Collaborate with other services across the PCN and neighbouring networks to promote prescribing quality and safety. Attend CPD, clinical, prescription, and dispensary team meetings and develop the role with the line manager. Adhere to organisational policies, confidentiality, safeguarding, information governance, health and safety, and lone‑working procedures. Qualifications – Essential Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council. Independent prescribing qualification. Qualifications – Desirable Membership of Primary Care Pharmacy Association (PCPA). Membership of the Royal Pharmaceutical Society. Clinical Diploma or equivalent (demonstrate experience). Experience – Essential Minimum of two years’ experience as a pharmacist demonstrated within a practice portfolio. Experience and awareness of acute and long‑term conditions in general practice. Experience of partnership/collaborative working and building relationships across organisations. Experience – Desirable Experience working in primary care. Experience working in a GP practice. Knowledge and Skills – Essential Minimum of 2 years post‑qualifying experience. Understanding of wider determinants of health. Appreciation of primary care prescribing and rational prescribing strategies. Knowledge of IT systems – word processing, e‑mail, internet. Planning, managing, monitoring, and reviewing general medicine optimisation issues for long‑term conditions. Ability to influence and motivate the audience to comply with recommendations. Knowledge and Skills – Desirable In‑depth therapeutic and clinical knowledge, evidence‑based practice.
