**About the Practice/Department/Team**
Primary Care Networks (PCN) are a group of local General Practice surgeries working in collaboration with one another to provide services to its local communities and patient groups. Lakeside Healthcare Group is responsible for 4 PCNs across 3 Localities and providing services to approximately 200,000 patients across these networks.
**Job Introduction**
This is a key role in data quality improvement and data reporting for the Primary Care Network (PCN) and has been newly developed specifically to support the team to deliver against key performance indicators; and to provide the PCN team and our sites with relevant information to support performance management, resource allocation, strategic decision making and contract compliance.
The post holder will support the PCN practice management teams to ensure data quality is improved to meet targets for both clinical and performance management data quality. This is to enable practices to have the necessary high-quality data and information they require to inform the provision of health care.
**What you will do**
- Carry out timely and accurate information analysis and report on agreed areas.
- Support and work with data management, clinical and management teams to improve data quality.
- Working with Information Systems Managers and practices assist the teams with the capturing of patient related data through the creation of templates, forms, and the use of 3rd party software.
- Support practice teams in identifying and targeting and coding patients for assessment, treatment, and recall.
**Essential Criteria**
- Demonstrable experience of administration work.
- Experience of data entry and validation.
- Experience of working in clinical or healthcare environment.
- Experience of working with clinical system, data base or patient records systems.
- Experience of data analysis.
- Ability to work in a team and independently.
- Calm approach to workload.
- Good attention to details.
- Able to work to agreed timescales and standards.
- Methodical and efficient in your approach to work.
- Ability to take a problem-solving approach.
- Able to prioritise own workload.
