This ongoing success has created a career opportunity within our Central Live-in Care Office for a **Care Needs Assessor**, covering the Central/ East Midlands area. We are looking for a confident, approachable people person, capable of developing positive relationships in the community with our clients and their families.

**Contract:
Permanent, Flexible Contract**
**Salary:
£11 per hour face to face time, dependent on experience plus £9.50 whilst travelling per hour**
**About the role:
As a Care Needs Assessor, you will be responsible for establishing a professional relationship with our clients, undertaking their first consultation, completing an initial assessment, and compiling the associated first care plan.
You will build and maintain relationships with clients, staff, and stakeholders and ensure in-date staff supervision, support, and training. This is a field-based role so you will need to travel within your designated region.
- To promote the services Agincare Live-in Care provides and undertake an initial assessment of care needs and associated risk assessments of potential new clients and create a care plan that reflects accurately and fully the client's care requirements, goals,and aspirations.
- You should expect to spend the majority of your working time in the community carrying out primary functions, but the role will typically also entail a certain amount of home-based administrative work
- To support care staff and the Regional Operations Manager by undertaking staff support activities such as spot checks, competency checks, and supervisions;
- To liaise with family, social services, and other healthcare professionals involved in client care where required to ensure that all relevant and accurate care needs have been considered.
- To use and continually update company systems and processes to record any concerns and to report to your line manager relating to significant changes in the service user’s health and support needs by working closely with the care team;
- To adhere to existing working practices, methods, and procedures and undertake relevant training and development activities.
**About you:
- Previous experience in Social Care (minimum 2 to 3 years);
- Assessment experience within Social Care, and a proven track record in care and risk management;
- You will be working alongside numerous Healthcare Professionals and will be required to travel to our clients in their own homes across your defined region. So, you will need your own transport and must be willing to travel some long distances when required.
- You will have knowledge of the social care sector and regulatory framework and be passionate about person-centered support;
- A Health & Social Care qualification is an advantage
- Full, valid UK / EU driving licence;
- Great communication skills - written and verbal;
- Highly organised, with a flexible approach
