Health and Wellbeing Instructor

We have an opportunity to recruit a Health and Wellbeing Instructor who will develop and deliver physical activity and healthy lifestyle management programmes, for service users with complex mental and physical health needs, embedding the principles of individual recovery, focussing on the whole person and building on people’s strengths and experiences.
The successful applicant will be required to work over a 7 day period on a fixed rota basis, with flexible hours, to maximise the support offered to patients, which includes weekend working.
**The successful applicant will be expected to**:
Promote, maintain or improve health and wellbeing by assisting individuals to adopt healthy behaviours. This will involve lifestyle assessment, setting goals for improvement which will support appropriate behaviour change.
Respond appropriately to referrals received from the Multi-Disciplinary Team across adult acute mental health wards. Be responsible for a designated caseload of service users with complex health needs. To work closely with Allied Health Professionals to ensure an MDT approach.
Work autonomously and independently to screen referrals, assess and provide interventions appropriately to Service User needs.
Have a working knowledge of local and national policies and legislation which govern current service provision.
Ensure effective communication pathways exist, acting as a lead within physical health and wellbeing to support the MDT in supporting Service Users recovery.

KEY RESPONSIBILITIES
CLINICAL
- To autonomously and independently screen, respond and triage referrals from the MDT
- To complete Health and Wellbeing Instructor initial assessments with service users who have been identified as having complex mental and physical health needs referred by the Multi-Disciplinary Team to explore and understand their health and wellbeing needs.
- To deliver a treatment plan for complex service users within the remit of health and wellbeing instructor, this may include 1:1s, group work and social reintegration.
- To manage and be responsible for a designated caseload of service users with service health needs while working closely and inputting to the MDT team.
- To collaboratively co-produce with service users specific goals and prescribe individually tailored health and lifestyle programmes. Whilst taking into account service users complex health conditions.
- To deliver a variety of evidenced based health and lifestyle interventions, whilst working towards national and local initiatives.
- Utilise outcome measures to evaluate the effectiveness of interventions as part of the service evaluation process.
- To work collaboratively with service users with a range of physical health and mental health conditions, including potential to work with patients with infectious conditions, including Covid-19.
- Complete & contribute where appropriate to robust risk assessment & risk management plans, deliver and promote positive risk taking where appropriate to improve quality of life and maximise potential within recovery goals.
- Supervise service users in exercise/gym sessions on the ward, in the gym and in the community.
- To effectively build therapeutic relationships with service users, while also having the ability to use de-escalation skills if exposed to potential incidents of physical and non-physical abuse.
- Through structured, regular review meetings, review each service user’s progress towards their collaborative, co-produced specific goals and ensure effective communication within MDT meetings.
- To ensure development and dissemination of good practice in the delivery of health and wellbeing programmes and to share expertise with the MDT.
- To refer service users onto other disciplines within the Multi-Disciplinary Team or other agencies as appropriate
- To actively participate in Multi Professional reviews and Team meetings.
PROFESSIONAL
- To participate in service innovation alongside Clinical Lead AHPs and Quality leads
- To respect and value the different therapeutic approaches within the Multi-Disciplinary Team and ensure that the agreed therapeutic approach is adopted and followed.
- Responsible for maintaining and conducting oneself in a professional manner towards service users, carers, colleagues and other agencies.
- Participate in joint working with the MDT as well as appropriate experts/agencies.
