An exciting opportunity has arisen to be part of a new and dynamic integrated multi-disciplinary team, SCOPE working with individuals with a learning disability and/or autism who have complex care requirements. The team will be supporting individuals from the age of 14 years and upwards at different life stages, from transition from school into adulthood, moving to their own accommodation and beyond into older age.
The team will be made up from disciplines from both Health and Social Care, such as social workers, Occupational Therapists, Speech and Language Therapists, Community Learning Disability Nurses, Day Opportunities, care providers and Psychologists.
SCOPE is leading in developing locally based provision, which will improve the lives of those with complex care requirements as well as their families and carers. The team will have a whole person approach and will embed the principles of positive behavioural support into the delivery of support.
The role of the Outreach worker will be intrinsic to this work, support individuals in their own homes as well as out in the community, inform assessment and support to implement positive behaviour strategies, which will improve the lives of the individuals as well as families, carers and the provider of care. Outreach Workers provide direct care and support to individuals residing across the whole of Perth and Kinross region.
As this is a new team these four posts will be at the forefront of developing the service and help shape how support is delivered to those with most complex care requirements.
The team will be centrally based in Perth with access to office accommodation in both the North and South localities.
**The Individual**:
We are looking for individuals who have experience of working with vulnerable people in the community, with preferred experience of work those with a learning disability and/or Autism. An understanding of individuals who are vulnerable or at risk and the care provision they require is preferable.
We are looking for individuals who have good communication skills, both verbal and written.
They also need to be able to cope under pressure and respond to the needs of individuals when they are display stress and distress behaviour.
To have the ability to work flexibility and adapt to the changing demands of the work.
To be able to work on their own but also work with well within a team.
To be compassionate in their approach and treat individuals with dignity and respect.
To be willing to develop their knowledge and skills to meet the needs of those who are the most vulnerable in society to ensure that the have better health and wellbeing outcomes.
To have or be willing to commit to undertaking and achieving a minimum of SVQ II in Health & Social Care
Will be required to register with the Scottish Social Services Council (SSSC) and responsible for maintaining annual membership.
Interviews are currently being undertaken virtually via Microsoft Teams and therefore access to a device to enable this will be required.
