Health Care Assistant/support Worker

**Job Summary**
As a Healthcare Assistant, you will play a vital role in providing compassionate and high-quality care to individuals in various settings, including care homes and home care environments. Your primary responsibility will be to support patients with their daily activities, ensuring their comfort and well-being while adhering to care plans. This position requires excellent communication skills and the ability to work effectively as part of a team.
**Duties**
- Assist patients with personal care tasks, including bathing, dressing, and grooming.
- Support individuals with mobility and physical activities, ensuring safety at all times.
- Help with meal preparation and feeding when necessary.
- Maintain accurate records of patient care and report any changes in condition to nursing staff.
- Follow care plans as directed by healthcare professionals to meet individual patient needs.
- Engage patients in social activities to promote mental well-being and companionship.
- Ensure cleanliness and organisation of the living environment for residents or clients.
- Drive patients to appointments or outings as required, ensuring their safety during transport.
**Skills**
- Proficient in English with strong communication skills for effective interaction with patients, families, and team members.
- Experience in a care home or home care setting is highly desirable.
- Familiarity with creating and following care plans tailored to individual needs.
- Basic IT skills for maintaining records and documentation accurately.
- A valid driving licence is preferred for transporting patients when necessary.
