Job Overview

We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will have a strong background in providing care and support to individuals in various settings, including home care services. This role involves assisting clients with daily activities, ensuring their comfort and well-being while promoting independence.

Responsibilities

  • Provide personal care and support to clients, including assistance with bathing, dressing, and grooming.
  • Administer medication as prescribed and ensure adherence to care plans.
  • Assist with meal preparation and dietary needs, ensuring clients receive proper nutrition.
  • Support clients with mobility and heavy lifting as required.
  • Engage clients in meaningful activities to promote social interaction and mental stimulation.
  • Implement behaviour management strategies as needed to support clients with dementia or Alzheimer's care.
  • Maintain accurate records of client progress and communicate effectively with healthcare professionals regarding any changes in condition.
  • Drive clients to appointments or social activities as necessary.
  • Collaborate with case management teams to develop and update individual care plans tailored to each client's needs.

Requirements

  • Previous experience in senior care, home care, or hospice care is highly desirable.
  • Knowledge of medical terminology and experience in patient care is essential.
  • First aid certification is preferred; training may be provided for the right candidate.
  • Familiarity with caregiving practices for individuals with dementia or Alzheimer's is an advantage.
  • Strong communication skills and the ability to work effectively within a team environment.
  • A valid driving licence is required for transporting clients when necessary.
  • Ability to perform heavy lifting tasks safely and efficiently.
