**About the post**
One of the NHS Long Term Plan ambitions is to improve the experience of care for Young People aged 18-25. Good relationships and protocols are essential to making this as seamless as possible for the CYP.
Transition can be a difficult time for young people and their families, because it is a lengthy process and involves various professionals and servic es. As part of the NHS Long Term Plan, Herefordshire and Worcestershire CAMHS are developing a multidisciplinary team consisting of Health staff and Professionally Qualified Youth Workers who are experts in supporting Young People’s personal, social and educational development.
We require a highly experienced, resilient and resourceful individual, who acts in the best interest of young people and families to ensure that they consistently receive high quality care when preparing to transition out of CAMHS. You will have a passion for person centred care and involving young people in service development and review. You will be able to demonstrate strong communication and problem solving skills to support your ability to work in collaboration with statutory and non-statutory agencies to ensure a seamless patient journey.
The CAMHS Team Leader will directly supervise the team to support & negotiate transitions to Adult Mental Health Services and Primary Care, leading multidisciplinary and multiagency meetings and leading on HR functions for the team. This is an innovative model where non-traditional staff roles will be utilised to ensure we have the best skills available to meet Young People’s needs as they approach adulthood.
You will be able to demonstrate
- Experience of working with the MDT and ability to contribute effectively to the clinical care and treatment for patients with mental health issues who are reaching Transition age
- An ability to develop systems, and experience of collating and interpreting data to support service improvement
- Adapt and develop Innovative ways of working in the best interests of the service and CYP and families
- An ability to lead a team.
