**Job description**

**_We are seeking a motivated Community Wellbeing Coach for this new role where no day will be the same. You will work alongside our community team, which includes a team of Social Prescribers who work with our local GP surgeries. Whilst the post has a focus on physical activity/movement the ideal applicant will view this as just one element to support people moving towards their optimum mental health and wellbeing. A key aspect will be supporting people back into their communities who may have lost confidence, mobility and motivation over the last couple of years._**

**_The role is person centred and will be delivered in an asset based way. Working together you will look at what matters to individuals in their lives and within their community. As you connect people with local resources, activities and services you will begin to bring about sustainable lifestyle changes._**

**Main Purpose of Job**: To support people to take steps to improve the way they manage their health and wellbeing, based on what matters to them - with a particular focus on enabling more movement/physically activity.

**Key Activities**:

- Provide one-to-one and small group health coaching support for people, based on what is important to them, with the aim of: improving people’s levels of ‘activation’, empowering people to manage their own health and improve their health outcomes.
- Undertaking assessment and action planning with individuals around “What Matters to Me”
- Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals. It is important that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the health and wellbeing coach role - e.g., when there is a mental health need requiring the patient be referred to an appropriately qualified practitioner.
- To support the development of a buddy scheme.
- Maintain files and information held about patients worked with and ensuring these are always up to date and held in accordance with the Company’s General Data Protection Regulation Policy.
- Work closely with your team to ensure that the relevant monitoring data is recorded to ensure requirements by funders are met and adhering to data protection legislation and data sharing agreements.
- Ensure that the staff team, local community organisations and local GPs understand the health coach role, how to refer to them, and which people may particularly benefit from health coaching.
- Promote and raise awareness of the health coaching service particularly to groups and communities that experience barriers to access.
- Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role.
- Participate and collect information that measures the impact of health coaching as an intervention, data required by funders and adhering to data protection legislation and data sharing agreements.

- Participate in regular supervision and continual learning.
- Know and adhere to Knowle West Health Park Company’s policies and procedures.
- Undertake any other duties appropriate to the grading of the post as required.

**Essential Criteria for role**
- NVQ Level 3, Advanced level or equivalent qualifications or working towards
- Coaching/counselling qualification/ experience or other relevant qualification/experience
- Using coaching approaches/frameworks and models or other helping strategies e.g., Motivational Interviewing, CBT or others
- Knowledge and skills in creatively providing a range of physical activities and understanding how this links with positive health and wellbeing
- Excellent one-to-one and small group facilitation skills including conflict resolution
- Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity
- Experience of data collection and using tools to measure the impact of services
- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers.
