Vulnerable Patients’ Coordinator

As a Vulnerable Patients Coordinator you are an integral part of the Special Allocation Scheme based in Bradford and Leeds.
The purpose of the scheme is to deal with patients who are aggressive and abusive, as well as, in some cases, violent. In doing so it aims to protect GPs and practice staff, and allows them to carry out their roles safely, in addition to protecting the safety of patients and other visitors.
All staff have the right to work without the fear of intimidating behavior, whether actual or perceived, and it is vital that any scheme commissioned to allow them to do so is robust and operates efficiently and in line with the regulations, whilst being appropriately supportive of the patient.
You will possess an excellent standard of literacy and numeracy, hold first-rate interpersonal skills and be able to communicate with compassion. Well-developed IT skills are a must to be successful in this post, along with an all-round working knowledge of Systemone, Microsoft outlook, Word and Excel.
You will be able to liaise with all members of the Bevan Clinical, Admin and Safeguarding practitioners and staff from other agencies across Bradford and Leeds.
- Administration_
1. Act as a point of reference for internal telephone queries, taking appropriate action personally or redirecting as necessary.
2. Produce reports and letters in accordance with service guidelines.
3. Issue repeat prescriptions and deal with prescription queries
4. Carry out general administrative and clerical functions including photocopying, filing and faxing as required.
5. Process the client referral system and associated paperwork in accordance with Bevan Healthcare systems.
6. Maintain effective and efficient systems and office procedures including adequate supplies of forms/leaflets and stationery requirements.
7. Maintain effective diary management as required by management.
8. Process incoming/outgoing post, taking appropriate action as necessary.
9. Maintain an efficient filing and records system, encompassing medical secretarial services.
10. Collate data information and prepare returns appropriate to grade as required by management.
11. Receive all SAS new patient notifications.
12. Collate all information required regarding patients on the scheme.
13. Send out initial introductory letters to patients on the scheme
14. Produce activity reports for the SAS detailing new registrations, leavers and recorded patient contacts.
15. Regular audit of SAS list to identify people who have left without permission.
16. To review registrations and gather information from previous GP, placing alerts on systems regarding the Special Allocation Scheme.
17. To contact every new patient to explain the concept of the scheme; introduce how it works, hopes for how it would be used and progression off the scheme (as behavioral changes are evidenced)
18. Arrange appropriate security cover for appointments, if required.
19. Complete SAS submissions

1. Ensure that comments/complaints are dealt with in accordance with the complaints policy.
- Technology_
1. Use and maintain current systems relying on information technology and to assist with implementation and development of new systems and information technology as appropriate to the grade. This will include word processing skills and the use of other software where appropriate.
2. To train new members of administrative staff on current systems and processes.
- Location Management_
1. To work as part of the administration team providing cover and support to colleagues within Bevan Healthcare during periods of annual leave or sickness to ensure a continuous provision of service.
2. Ensure the security of locations and other buildings including equipment as required.
- General_
1. Any other duties as may be appropriate to achieve the objectives of the post or to assist the service in the fulfilment of its objectives commensurate with the post holder’s grade, abilities and aptitude.
- Policies & Procedure_
1. The post-holder will be required to comply with all policies and procedures issued by and on behalf of Bevan Healthcare. In addition, if the post-holder is required to work at other site they must adhere to the specific policies relating to the premises in which they work.

- Clinical Governance & Risk Assessment_
1. The post-holder must be aware of and ensure compliance with the Bevan Healthcare Clinical Governance systems and Risk Management Programme.
