Frailty Administrative Coordinator

The frailty admin coordinator will support team with completion of referral forms for social services and community services.
They will support the team in recording data for quality improvement and reports.
They will assist with minute taking and meeting preparation.
They will assists in the preparation of events related to the frailty/UCR and virtual ward developments.
They will be able to order equipment via IPROC as required, and be able to maintain IT equipment.
The will work with the team to showcase data and promote the frailty service, e.g newsletters, Intranet app, education displays.

The Frailty team is a successful, dynamic service which supports the needs of the frail population.

The team is multi -disciplinary and works closely with partner organisations in order to support patients receiving right care in the place. We are passionate about ensuring the frail patient needs at met and we work with patients, families and carers in order to deliver high standard of care.

3, Be always professional.

4, Be an excellent communicator.

5, Maintain dignity and respect for patients.

7, Complete assessment forms based on information obtained in electronic patient record and liaison with the specialist team.

8, Liaise with community services, social services, and GPs to ensure the patient information is communicated.

9, Be proficient in using patient electronic records e.g. EPR and System one.

10, Be proficient in word, excel and PowerPoint as well as minute taking to support the frailty service.
