This role focuses on delivering exceptional care to patients with complex needs, including frailty and heart failure, while supporting safe discharge and reducing avoidable hospital admissions. Key Responsibilities Clinical Delivery Provide comprehensive assessment, diagnosis, treatment, and evaluation for Virtual Ward patients. Lead on heart failure management within the service. Independently prescribe and review medication (including IV prescribing). Identify early deterioration, acute illness, and frailty needs using advanced clinical reasoning. Support end-of-life care planning and guide patients, families, and care homes. Manage acute and chronic conditions using evidence-based interventions.Care Coordination Work collaboratively with acute trusts, community teams, specialist nurses, therapy teams, primary care, and social care. Facilitate safe discharge pathways and integrated care planning.
