We are currently looking to appoint Band 6 Health Visitors across Wantage and Faringdon Oxfordshire.
In addition to our universal offer, we have many exciting and rewarding features within our service.
A few examples include health visitors being able to support maternal mental health, as well as leading on the delivery of perinatal mental health support groups, a teenage pathway led by health visitors working closely with our Family Nurse Partnership Team, breast feeding champions who ensure we achieved BFI stage 3 and strive to maintain the high standard of breastfeeding support we offer, and Chat health parent messaging service, to name but a few.
You will be leading the delivery of the Healthy Child Programme as part of a team that includes Nursery Nurses, Community Staff Nurses and Administrative Support Workers, with additional support from experienced Locality Team Leaders.

DUTIES AND RESPONSIBILITIES
**Clinical**:
Establish and maintain partnerships with families and other agencies in multi-agency and
integrated working processes, identifying the health needs in a defined population and targeting
resources to address these.
Assess, plan, implement and evaluate individual/ family focused/group programmes of care,
working in partnership with other agencies, including early years’ provision.
Adhere to the Oxford Health Healthy Child Programme frameworks and pathways in relation to
our client group.
Be conversant with and work within local safeguarding, looked after children, domestic abuse
and vulnerable children policies, procedures and advice and provide support to the team
regarding these.
Plan, deliver and evaluate public health initiatives through community-based health promotion
activities, which reflect need and meets the key performance targets of the organisation.
Be prepared to undertake nurse prescribing in response to assessed need, in accordance with
Trust policy and in line with NMC code for non-medical prescribers.
**Professional**:
Maintain accurate records and adhere to all aspects of the Oxford Health Information
Governance Policy, Caldecott guidelines, The Data Protection Act 1998, NMC Code, NMC
Standards of Proficiency for Specialist Community Public Health nurses, and other organisational
requirements in perpetuity and raise any concerns regarding these with your line manager.
Participate in all relevant aspects of clinical audit at national and local level.
Keep updated with and provide evidence-based practice.

**Leadership**:
Lead in recruitment, selection and retention of skill mix staff. Plan induction programmes for all
new team members, in conjunction with the Locality Team Leader.
Line manage skill mix staff, being responsible for their performance and development and accountable for delegating pieces of work assessed as appropriate to their skills and job
description.
Identify skills of team members and support their ongoing professional development.
Disseminate evidence-based practice as identified in National Frameworks and guidelines.
Develop and help maintain systems within the workplace to promote maximum organisation and
efficiency of the team.
