Full Time (37.5 hours per week),Fixed term - 6 months
Salary £19,300 to £20,250 pa rising to £24,300 through length of service, plus brilliant benefits

We have a fabulous job opportunity for you to join our awesome team. We support our brilliant customers who have been referred to us through the NHS crisis and psychiatric liaison teams to support them with Housing related or social needs to relieve some pressureto the customers whilst they are at crisis point and allow the Crisis teams to focus on the customers mental health.
It’s a fast-paced short term service supporting our customers virtually and in the community across County Durham and Darlington. As a team we are super passionate about our work and have developed some amazing working relationships with the Crisis and psychiatricliaison teams and because of that we are now expanding our team.

**About you**
You’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.
You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. You’ll create bespoke and personalised support plans with and for your customers.Each plan willlook to meet our customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.
