To assist registered nurses/practitioners to deliver care to patients/clients, working in partnership with other health care professionals, the patient their family and carers.
Deliver patient care as documented in the nursing care plan. Carry out a wide range of duties under the direction of a registered nurse for the period of the shift.
Applicants should be enthusiastic with an awareness of and committed to the modernisation of the service. They should be able to work well within a team and also alone.
You must be able to travel between patients homes both within towns and rural areas in a timely manner.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Undertake direct patient care tasks during a patients visit to the outpatients department e.g. weighing of patients, measuring heights, BMI, urinalysis recording blood pressure and reporting any abnormalities to the nurse in charge.
Assist the registered nurse or other team member in carrying out nursing procedures or activities.
