**Description**:
We have an exciting opportunity for an experienced and driven for a part time Communication and Partnership Coordinator Together is working to provide this new service which is part of an exciting project delivering support for SMI (Severe Mental illness) clients for their Annual Physical Health Check across Norfolk and Waveney which will include supporting the team in communications, engagement and networking aspects with stakeholders as well as people with SMI.

The project is looking to promote and support Norfolk and Waveney workforce to build understanding about the physical health risks facing people living with an SMI, increased likelihood of key health conditions and reduced life expectancy. You will also support people with an SMI to engage with their health check and interventions and/or lifestyle changes which will reduce the health inequalities noted for this population group.

As our Communication and Partnership Coordinator, you need to possess excellent organizational skills, strong verbal and written communication skills. You should also have a proactive and problem-solving attitude. Building on an already solid foundation of partnership working, you will use your stakeholder management skills to foster strong networks with GP Practices and other mental health providers to ensure the service is well connected to organisations and agencies providing support in Norfolk and Waveney.

You will also work in a person centre way to support people with SMI to engage in their Physical Health Check appointment by accompanying them to the appointment if needed as well as follow-up on feedback or any relevant necessary information after their physical health check such as signposting to appropriate support. Outcomes-focused and experienced in supporting a diverse team, you will ensure the efficient running of the service in line with Together’s ethos and policies.

This post is part time (22.5 hours per week) and fixed term until March 2026.

- Support a greater understanding of the importance of good physical health
- Support a greater understanding of the additional challenges faced to the physical health for those living with an Serious Mental Illness, and increased risk of early mortality
- Understanding what the Physical Health Check entails
- Increase awareness of entitlement to the annual Physical Health Check, physical healthcare services and interventions
- Empower individuals to exercise choice and control, offering a person-centred service led by the wishes of the individuals you are working with
- Seek to safeguard individuals within the safeguarding adult’s guidelines and procedures whilst promoting independence

**Specific**
- Engage creatively and effectively with this group with support to ensure they maintain ongoing access to the physical health check and/or flu and covid vaccination at their GP practice
- Promote the project and SMI Physical Health Check via attending various events, meeting, sessions as a stall holder/speaker.
