An exciting development opportunity exists for the post holder to take primary responsibility for the facilitation of Stroke MDT meetings, ensuring they run effectively and are well structured. They will ensure that all patients referred are discussed in line with stroke MDT operational policies.
Providing administrative support to the stroke multi-disciplinary team meetings, this includes preparing and maintaining all related documentation and information to support the stroke MDT database. Ensuring patient treatment plans and multi-disciplinary team decisions are recorded in to the designated stroke database, with appropriate updates when relevant actions have been executed.
The post holder will also initiate and maintain the local, Stroke Sentinel National Audit Program database. All targeted data entries will be made timely ensuring data accuracy at all times. This will involve monitoring and analysing the Trust’s performance against the SSNAP targets. In order to aid service development you will provide and present support data relating to Stroke Team performance at a local and National level including data extraction to highlight fluctuations.
Alongside the stroke nurse coordinator service the post holder will maintain the Stroke Admissions Register, referrals and Stroke Association logs to ensure a seamless safe and effective service provision.
We are looking for a dedicated, hardworking and enthusiastic individual to enhance and support our passionate, committed and effective Stroke Team. To work in close collaboration with the Stroke Clinical Lead, Stroke Lead Nurse, Stroke Ward Managers, Lead Therapists and the Stroke Nurse Coordinators service. The role demands a high level of accuracy and attention to detail and understanding of the relevant audit and data base requirements.
Knowledge of patient administration system Cerner and the SSNAP Audit would be an advantage as this post involves facilitation of the MDT meetings; ensuring patients are managed along the stroke pathway in accordance with local and National Guidelines for Stroke management including the Stroke Sentinel National Audit Program.
The successful applicant will possess excellent communication and interpersonal skills will be efficient at organising and prioritising a busy workload and remain calm under pressure; they will have strong regard and appreciation of patient confidentiality.
The Countess of Chester Hospital NHS Foundation Trust consists of a 600 bedded large District General Hospital, which provides its services on the Countess of Chester Health Park, and a 64 bedded Intermediate Care Service at Ellesmere Port Hospital.
