Dynamic Support Facilitator

Across Buckinghamshire there is limited provision of intensive support for CYP with a learning disability and/or autism between CAMHS Tier 4 inpatient and universal CAMHS Tier 3 provision.
To support this, Buckinghamshire Clinical Commissioning Group (CCG) and CAMHS developed a business case to support CYP and families living in Buckinghamshire with Facilitator support and pilot this over 36 months.
Initially the Facilitator support will be provided to children and young people who are inpatients or at risk of being admitted to hospital. support Facilitator will also be extended to the most vulnerable children with a learning disability and/or autism, including those who face multiple vulnerabilities such as looked after and adopted children, and children and young people in transition between services.
The Facilitator will support the whole ‘System’ to deliver high quality, effective and efficient service, and system responses to:
- Improve the experience of children & young people
- Improve the experience of Parent/carers & families
- Improve Access
- Improve Quality of care
The Facilitator will work across the local system with and for the family and the children and young people to ensure with the most complex needs, initially those at risk of admission or in hospital, receive appropriate personalised, integrated support.
These exciting new roles will form part of a national Keyworker Pilot developing new ways of working in partnership with NHS England, Buckinghamshire Integrated care system and CAMHS. Importantly the pilot will be driven by the lived experience of young people and their families, therefore Buckinghamshire Parents Carer forums involvement are critical partners to shaping how we work in the future.

The Facilitator will work across the local system with families, to ensure children and young people with the most complex needs, initially those at risk of admission or in hospital, receive appropriate personalised, integrated support. CAMHS and the Lead Commissioner for children and young person will support the post holder to deliver the Facilitator function.
Working Environment
- The central base will be Aylesbury with travel around Buckinghamshire to deliver link worker role/training/assessment and for joint meetings.
- We currently work using a hybrid model with the majority of work being remotely, however there are days that the team meet on-site and this is scheduled in advance. The post holder will have access to an open plan office with other team members, and they will have administrative support. There will be access to rooms for supervision.
- There is a need to use effective and efficient keyboard skills
GENERAL
- To contribute to the development of best practice within the service.
- To contribute to the promotion of Buckinghamshire CAMHS through written and oral means to stakeholders inside and outside of the Trust.
- To maintain up-to-date knowledge of legislation, national and local policies, and procedures in relation to Mental Health and Primary Care Services.
