We’re looking for passionate and dedicated people with a talent to work with vulnerable adults with a wide range of issues.
**Role Overview**
This role supports the resettlement of individuals with a rough sleeping history who are hardest to reach by facilitating access to accommodation. The position involves close collaboration with lead workers, housing providers, and statutory/non-statutory agencies to ensure a smooth transition into stable housing.
**Duties & Responsibilities**
- Build and maintain strong relationships with accommodation providers.
- Monitor bed space availability and match clients appropriately.
- Ensure timely and suitable offers of accommodation are made.
- Collaborate with lead workers to ensure all necessary steps are completed before move-in:
- Coordinate with housing options teams and ensure up-to-date information is shared.
- Assess the need for social care involvement.
- Ensure support and risk assessments are current to avoid duplication.
- Match clients to accommodation based on individual needs and preferences.
- Ensure a personalized approach to maximize the chance of successful resettlement.
- Continue engagement post-move-in to monitor progress and identify additional support needs.
- Case Management (When No Other Services Are Involved)
- Take on the role of case holder.
- Refer clients to appropriate services to build a support network.
- Conduct needs and risk assessments.
- Support clients in developing skills and confidence to live independently.
- Promote safety, security, and well-being throughout the resettlement process.
- Participate in meetings and contribute to data analysis.
- Work collaboratively with other agencies to enhance service delivery.
