Providing intensive one to one support to individuals to access statutory assessment and supporting the development of a holistic and person centred strength-based personalised plan according to the needs of the individual.You will work as a single point of contact connecting the Nottinghamshire Healthcare Trust Nottingham City Councils Housing Solutions service and accommodation providers to assist with the process of acquiring and then resettlement into accommodation following discharge from hospital.About YouThe ideal candidate will have:Knowledge of local social and healthcare services and an ability to navigate systems and pathways into servicesExperience of supporting individuals with complex needs (e.g. rough sleeping substance misuse physical and mental health needs)Understanding of the wider issues impacting upon homelessness the barriers facing people with multiple and complex needs and the challenges involved in resolving rough sleepingEvidence of working successfully in partnership across different sectors building and maintaining good working relationshipsAbility to advocate and negotiate on behalf of citizens and to give independent adviceKnowledge of housing legislation rights and responsibilities in both the public and private sectorsA DBS enhanced check is required for this post for a regulated activity.You can find the job description for this post hereAt Nottingham City Council we believe that work is what you do not where you do it.
