**CONTRACT**:Maternity cover, secondment/fixed term for 9 months

**Job Summary**:
The role is a 9-month maternity cover, secondment/fixed term post and will join the educational leadership team at Islington GP Federation.

The Personalised Care Workforce Education Lead will be responsible for shaping workforce planning, education, development and support for personalised care service teams based in general practice across Islington, including:

- Social Prescribing Link Workers
- Health and Wellbeing Coaches
- Care Coordinators.

This is a strategic role that will work closely with local providers and colleagues across North
Central London (NCL) to review how primary care in Islington supports patient autonomy and the delivery of the Personalised Care agenda as outlined in the NHS Long Term Plan.

With support from PCN Clinical Directors, the Federation operational team and Islington
Training Hub, they will lead, and significantly contribute to, service development activities.
The role will advocate for new workforce models that promote at scale integrated working to support population health need and seek to address workload pressures on general practice.

**About Islington GP Federation**

Islington GP Federation (IGPF) is a rapidly growing organisation representing 31 practices located in Holloway Road. The Federation is an established leader driving forward new ways of working; running Islington’s extended hours service (I: HUB) and supporting the operational delivery of Primary Care Networks (PCNs) and Quality Improvement work. Our current range of services includes I: HUB, Community ENT, Community Gynaecology, practice-based pharmacists and a range of practice support mechanisms.

IGPF works closely with a range of partners including the local commissioning group, NHS
England, Healthy London Partnership, Public Health, local trusts such as the Whittington
Health and C&I the local authority.

GP practices have been working collaboratively as eight integrated care networks since 2017 and in 2019 reconfigured to five Primary Care Networks (PCNs). Islington’s PCNs form the foundation for the delivery of integrated care which is locally-focused and responsive to improving patient outcomes across the borough. Each PCN is led by a highly effective management and support structure which includes clinical and operational leads who are responsible for the service delivery of priority programmes such as First Contact Practitioner
MSK and Paramedic pathways, Pharmacy, Care Homes and Personalised Care.

An exciting opportunity is available for an individual to lead the workforce and service development to support the First Contact Practitioner (PCP) MSK pathway in primary care across four of Islington’s PCNs.

**Responsibilities**:
The Personalised Care Workforce Education Lead will work to:

- Coordinate and plan training and development opportunities for Personalised Care teams based in general practice as required, including undertaking annual training needs assessment.
- Work with Clinical Directors, Federation, Training Hub and Operational teams to design personalised care service to address patient health needs and general practice working.
- Lead on service evaluation against KPIs to track progress against programme objectives.
- Identify and recommend to Clinical Directors and Federation solutions to improve value and efficiency of personalised care service teams to support general practice and patients.
- Support general practice engagement with the personalised care agenda and borough services.
- Work with Federation operational teams to maximise access to relevant data to support workforce planning and reporting, including information governance.
- Develop strong working relationships with external stakeholders at various levels to monitor standards with borough providers and users, including review of local service agreements.
- Attend Federation, borough and NCL level meetings on a regular basis and provide expert guidance to support operational delivery of personalised care services.
- Facilitate PCN education sessions to develop an understanding of workforce planning, development needs and service priorities.
- Participate in borough induction and events as required.
- Present to relevant boards and Clinical Director meetings as required.
- Support service development and PCN wide protocols for referrals, performance monitoring, CPD and training and clinical/educational supervision for three personalised care roles.
- Facilitate national, regional and local communications to PCNs, practices and borough providers on professional learning activities and service updates.
