Due to retirement, a vacancy is becoming available for the Clinical Lead of the Community Tissue Viability Service. We are looking for an experienced Tissue Viability Nurse with leadership and management experience. This is a dynamic team with a clear focus on assisting services to deliver high standards of care in the prevention and management of skin and tissue damage.

This county wide service supports all Oxford Health Community Services, General Practice and Care Homes within Oxfordshire through the provision of specialist assessment and advice, an annual program of study days, facilitating link nurse forums, and the development of clinical guidelines and care pathways. The service has introduced new ways of working in response to the changing environment within community services and the consequences of the Covid-19 pandemic. The are further plans for the service to be pivotal in the post Covid recover plans for district nursing services. We are looking for a visionary individual with the skills, abilities and experience do be able to deliver on this vision.

The Tissue viability Clinical Lead will provide specialist expertise directly to patients and provide advice, support and education to staff working in a range of clinical settings. They will make autonomous clinical decisions and implement care which may include the prescribing of advanced treatment modalities or the direct referral of patients to other specialist services. They will carry out assessments of patients with complex needs using a range of advanced skills including history taking, health needs analysis, and interpreting diagnostic tests.

A primary purpose of the role will be to provide clinical leadership to a wide range of teams in respect of wound prevention and management, to monitor standards of care and contribute to quality improvement in respect of wound prevention and management.
The Advanced practitioner will work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health outcomes and healthcare delivery.
