**PART 1**: JOB PROFILE 1. Main Purpose of Job This role will support the Transition Lead and Clinical Community Nurse and contribute to: a high quality and effective support/social group. To assist in providing a high level of specialist palliative care, encompassing a supportive and holistic approach to the child/young person with a life limiting condition and their family in the community environment. To work flexibly to cover a 24/7, 365-day service.
The post holder will work alongside the multi-disciplinary team and maintain the delivery of care in line with the Hospice philosophy. 2: Position of the role in Claire House The post holder reports to the Transition Co-ordinator and is ultimately accountable to the Director of Clinical Services. 3: Scope of Job To provide support in social sessions for young people and their families. To act as a contact worker demonstrating good core communication skills depending on the specific needs of the child/young person and their family.
To manage their own caseload ensuring that young people and families receive safe and effective care to meet their need. DMD caseload/Social Groups/Parental link. Monthly social groups, friendship groups and residential trips. Supporting social group with appropriate clinical skills.
To contribute to service development across Transition. 4. Dimensions and Limits of Authority To work under supervision ensuring practice is in line with Claire House policy and procedures. Appropriate advice should be sought from the Transition Coordinator and Clinical Community Nurse.
The post holder will be expected to be responsible with financial expenses seeking appropriate sign-off from a budget holder before purchases are made. 5. Qualification and Skills Level Proven experience of providing support for children, young people and adults. Excellent interpersonal and communication skills.
**Professional background NVQ Level 2/3 or equivalent. Part 2**: MAIN DUTIES AND RESPONSIBILITIES 1. Community Health Care Support Worker To provide social sessions for young people who attend Claire House across two sites and where appropriate or in residential settings. To act as a positive role model.
Demonstrating CH Values and Behaviours. To receive and undertake regular supervision To undertake duties to a satisfactory professional standard in accordance with statutory requirements and the policies and procedures of Claire House. To contribute to the development of the Young Adult Team in accordance with policies set out by the head of service. To maintain and develop professional expertise.
To work collaboratively with volunteers to ensure that their contribution enhances the quality of service provision. To attend regular I: I and caseload review sessions with the Young Adult Team Leader. To undertake any other duties which are consistent with the post. 5: General Claire House Requirements Adhere to and comply with organisational policies and procedures at all times.
Take all reasonable steps to manage and promote a safe and healthy working environment which is free from discrimination. Comply with the organisational policy on confidentiality, and the Data Protection Act 1998 as amended relating to information held manually or on computerised systems. Respect the confidentiality and privacy of clients and staff at all times. Maintain a constant awareness of health, welfare and safety issues affecting colleagues, patients, visitors and themselves, reporting any accidents or fault in line with the organisational policy.
Participate in personal training, development, and appraisal and attend all relevant training courses as required. Fully participate in health and safety training Adhere to and comply with organisational policies, procedures and guidelines at all times Implement risk management strategies, taking all reasonable steps to manage and promote a safe and healthy working environment which is free from discrimination Implement Health and Safety regulations through risk assessment. Maintain a constant awareness of health, welfare and safety issues affecting colleagues, service users, volunteers, visitors and themselves, reporting any accidents or faults in line with organisational policy. Participate fully in health and safety training The post holder must act at all times in a professional and responsible manner and have due regard to confidentiality and Health & Safety legislation.
