Guild Care is on the lookout for a dynamic **Support at Home after Hospital Coordinator** to join our incredible Community Services team
- Location: Worthing, West Sussex
- Salary: £13.09 per hour
- Hours: 14 hours/week, Thursdays & Fridays
Essential:
- Passion for supporting older people to live life to the full
- Coordination skills to lead a team of awesome volunteers.
- Understanding of the needs of older people.
- IT savvy with MS Office skills.
- Great interpersonal and communication skills.
- Ability to maintain confidentiality.
- Satisfactory DBS check.
Desirable:
- Knowledge of local organisations supporting older people.
- Experience with volunteers and understanding family and friends' carer issues.
- Familiarity with legislation and codes for older people's services.
Your Mission:
Coordinate a volunteer team providing crucial support to older people post-hospital discharge, promoting recovery, rebuilding confidence, and reducing re-admission risk. Be the positive force that ensures compliance, working seamlessly across Worthing and Adur.
What You'll Do:
- Recruit, lead, and support an amazing volunteer team.
- Prioritise and provide timely support post-hospital discharge.
- Build relationships with local groups and stakeholders.
- Assess needs, minimise risks, and refer for additional support.
- Contribute to monitoring and improvement initiatives.
- Promote Guild Care services and be an ambassador for positive change
