**JOB SUMMARY**
The role will work within an integrated care system to develop the pathways centring on the UTC and urgent care in Hillingdon. Using a change management approach and methodology to identify, plan and deliver transformation initiatives to support and enhance patient care and experience and improve efficiency and effectiveness.
**Primary Responsibilities**
**Person Profile**:
- Dynamic and flexible focused on delivery and outcomes. With an ability to approach complex problems in a pragmatic fashion;
- Be a non-tribal leader and team-player and able to move forward this exciting agenda across different teams, agendas, perspectives and organisations;
- Ability to work and deliver co-production and be able to bring along people and stakeholders on the same day episodic care journey;
- Track-record in delivering and managing complex programmes of work in urgent care; of owning implementations; and transformation and process improvement outputs in a real-life setting;
- Excellent transformation professional who possesses excellent communication skills and also able to be analytical when required; and
- Ability to put structure and clarity whilst working in developmental and agile approach.
**Key Duties and Responsibilities**:
The duties and responsibilities will include, but will not be limited to, the following:
- To develop and implement the Plan for improving UTC out of hospital pathways
- To continually improve UTC out of hospital pathways and those of the wider urgent care system in Hillingdon
- To be a Confederation representative on the HUECA Alliance Board with particular responsibility on all transformation and improvement of patient pathways into and out from the UTC / ED
- To contribute to the development of all other UTC matters in particular how these impact on such pathways into and out from the UTC
- Senior member of The Confederation Transformation Team who will be responsible and accountable for the design and implementation of the Urgent Care strategy and development
- Senior member of the integrated UTC and ED management team, recognising that the UTC sits in a critical junction of urgent pathways in a system; and that how those pathways perform and behave in that system, is impactful on the UTC itself.
- To develop new initiatives around urgent care in Hillingdon such that pathways are more integrated and effective for patients and improve the impact they have on primary care. To include (but not exclusively) broad aspects of same day episodic care, GP out of hours, NHS 111 and others.
- To work within the wider HHCP strategy in Hillingdon and to influence with HHCP how that strategy develops in the areas of urgent and same day episodic care
- Work with GP practices and Primary Care Networks (PCNs) to understand the impact they have on the UTC and wider urgent care, and the impact changes in these functions have on the practices and PCNs; to ensure where possible win-win developments; but always informed communications
- To work closely with the Confederation Operations team to develop pathways, implement change and ensure smooth handover from transformation workstreams to business as usual delivery, performance and reporting.
- To support the Operations team in delivery of the service by supporting the interface between the two organisations to achieve a seamless service for patients
- Using patient data at specific and general levels to evidence issues and generate proposals and to drive transformation and continuous improvements
- To ensure developments under the remit of this role are informed by the Hillingdon patient voice
- Prioritise initiatives in complex and nuanced environments
- Develop business cases and development proposals
- Facilitate and enable transformational change - build excellent functional relationships with and between multiple stakeholders
- Identify process improvement and where possible streamlined ways of working
- Utilise project management and change management methodologies to manage and deliver
