**Job details -**

To deliver a person-centred, wrap-around outreach support service, actively case-working complex needs and vulnerable housed individuals to prevent rough sleeping amongst prison leavers. To work in partnership with South Gloucestershire homelessness prevention services and identified Prison Resettlement services, NPS Probation services, police, and specialist local area housing support providers.

This role will be based in Kingswood and entails travel to other South Gloucestershire areas with some remote home working.

**You should have -**
- Knowledge of the criminal justice system, prison leaver resettlement and or Housing Options process.
- The ability to build effective relationships with clients with complex support needs and specialist community services in order to develop pathways to enable clients to access and sustain accommodation.
- Experience in working with people who are involved in the criminal justice system.

**Job Types**: Full-time, Permanent

**Salary**: £22,440.00-£23,812.00 per year
