An opportunity has arisen for a passionate, innovative individual to take on a new, novel role as Midlands-wide lead for Children’s Continuing Care. The postholder will take responsibility for working with the 11 Integrated Care Systems across the region to ensure Children’s Continuing Care is delivered effectively and equitably, in accordance with nationally set specifications and eligibility criteria.

The role has been situated within the West Midlands Children’s Network team, hosted by Birmingham Women’s and Children’s NHS Foundation Trust. This is to promote cross-working with relevant, interdependent network and clinical teams and to enable improved visibility of end-to-end pathways of care for children and young people with complex health needs, who often require Continuing Care. The postholder should expect to advocate for the needs of these babies, children and young people (and their families) which differ significantly from the majority of adults with Continuing Care needs. The postholder’s role will extend across the entire Midlands region.

The postholder will work as part of the NHS England All-Age Continuing Care team to ensure alignment with the wider Continuing Care programme. This postholder should have strong interpersonal and communication skills, and should expect to work with a broad range of stakeholders across provision, commissioning and patients/families within the role.

**Specifically, the postholder will**:
Lead and provide capacity and support for national and regional AACC projects.
Support the Head of Clinical Quality & Workforce Development in managing the agreed programmes of work in relation to CYP Continuing Care and CYP transformation.
Contribute, with regional team colleagues, to the ongoing national development of the operating model and priorities for delivery of the AACC Vision.
Advise on and be a key stakeholder in developing assurance around children’s continuing care with systems.
Develop collaborative working on children’s continuing care with other key stakeholders in the Nursing Directorate and with other stakeholders inside and outside NHSE, including Medical Directorate, Specialised Commissioning and regional CYP transformation Forums; and with Local Authority stakeholders in children’s social care and education.
Support AACC contribution to development and maintenance of safe and effective discharge processes in the region.
