Support the planning, delivery, and monitoring of PCN transformation projects aligned with NHS priorities and local health needs Coordinate project meetings, workshops, and events including agenda setting, minute taking, and follow-up of actions Monitor project timelines and task progress, escalating risks or issues where necessary Maintain and update project documentation such as plans, reports, risk logs, and communications Support project evaluations by gathering and analysing feedback, usage data, and outcomes Stakeholder Engagement Build relationships with GP practices, clinical teams, social prescribers, carer leads, and community partners Communicate clearly with internal and external stakeholders to support project awareness and participation Assist in the development of communications and promotional materials for transformation projects Ensure that patient and public involvement is considered in project delivery, where appropriate Service Improvement Support Identify areas for service development through data analysis, feedback, or stakeholder consultation Work with team leads to support continuous improvement approaches (e.g. PDSA cycles, process mapping) Contribute to the writing and editing of improvement plans, business cases, and evaluation reports Administrative Support Provide general administrative support to the PCN team, including document preparation, scheduling, and report collation Support the tracking of key project metrics and reporting requirements to ICS or funders Maintain confidentiality and adhere to all NHS data governance and information sharing protocols Confidentiality In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. Health & Safety The post holder will assist in promoting and maintaining their own and others health, safety and security as defined in the Practice Health & Safety Policy, to include: Using personal security systems within the workplace according to Practice guidelines.
Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks. Making effective use of training to update knowledge and skills. Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards. Reporting potential risks identified.

Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance. Work effectively with individuals in other agencies to meet patients needs. Effectively manage own time, workload and resources. Communication The post holder should recognise the importance of effective communication within the team and will strive to: Communicate effectively with other team members.
