Primary Care Mental Health Practitioners (Mental Health Additional Roles Reimbursement Scheme) - Bradford PCN4 Primary Care Network (PCN) This role is an exciting opportunity for a registered band 6 mental health professional (Nurse, Social Worker or OT) to work in a dynamic way across primary and secondary care to bring together mental health pathways and support the understanding of mental health within primary care. The role will provide holistic assessment of services users linking people into the community and VSCE resources as well as providing seamless transition into secondary care if it is needed. The role will also form part of multidisciplinary meetings held by GP Practices within primary care. **Closing date**: 06/05/2025 **Shortlisting date**: 07/05/2025 **Interview date**: TBC To actively promote better understanding and knowledge of mental health and wellbeing within primary care to enable the MDT to provide integrated and holistic offers of support.
