Mental Health Practitioner Closing date: 30 December 2025. The Proactive Care Team is a new team that aims to develop and deliver a community‑based integrated model of healthcare for individuals with the most complex health needs. The team will work in the three Community Partnership areas of the Central Locality in Bradford and Keighley. Using a Lead Provider model, BDCFT will work in collaboration with key partners including Bradford Teaching Hospitals NHS Foundation Trust (BTHFT), the GP Federations and voluntary sector partners to create a proactive, culturally competent and aware multidisciplinary team (MDT). Team members will identify those with the highest need and deliver a blend of preventative and recovery‑focused interventions in the community, supporting a reduction of premature mortality and improving outcomes for those who are frail or close to end of life. The overall aim of the Proactive Care Team is to help reduce health inequalities for those with the most complex needs, support a reduction in Accident and Emergency Department (AED) attendances and unplanned admissions, and improve access to community‑based provision to sustain good health outcomes. Work as part of a specialist multidisciplinary health team. Provide holistic mental health assessments, effective interventions, signposting and liaison with relevant agencies.
