Salford is currently undergoing service transformation to redesign our Home First discharge processes and pathways. Our vision for Home First is 'People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary'

We have established a Home First Multidisciplinary Community Hub to support people admitted to hospital to be discharged 'Home First' and we are looking for a nurse to join the team as a Home First Care Home Liaison Officer. You will work with therapy, nursing, pharmacy, social work and voluntary sector colleagues to plan discharge assessments and post discharge support. Your role will be to lead on promoting and strengthening the interface between Hospital and Care Homes, working closely with Care Homes and the Care Home General Practice. You will ensure effective Trusted Assessment for people moving to a care home from hospital in Salford.

**_The Service operates over 7 days, currently covering the hours of 8am to 6pm_
