Come and join Salford Care Organisation (SCO) for an exciting opportunity to work within our Intermediate Tier Services. This will be an exciting challenge and will give you the opportunity to play a key-supporting role in the development of the services. You will be working as part of a large multidisciplinary team that includes Physiotherapists, Occupational Therapists, Social Workers, Care Support Workers, Housing Officer, Assessment Officers and Administrators. Working as part of an integrated system with our other Community Services, this is an excellent opportunity to work as part of an innovative team, supporting SCO to deliver operational excellence and deliver a model to support discharge planning at the point of admission (a true Home First Approach).
**Main duties of the job**:
We are looking for a enthusiastic motivated individual's to join our Community Reablement Home First Team to assist the Therapy Healthcare Practitioner to carry out personalised care and recovery in the person’s own home. To provide support and person focused re-ablement as required to help ensure the efficient and effective running of the service.
To provide excellent customer service and establish and maintain communication between individuals, relatives, carers and the multidisciplinary team. To provide holistic patient care, correctly undertake tasks that have been delegated and to record information accurately and pass it on to the relevant people in the team in a timely manner.
Due to the role working in the community a Full Driving Licence and the use of own vehicle is essential.
You will possess excellent communication skills and have a proven track record of strong organisational skills. You must be able to work on your own initiative to organise and prioritise your own workload and balance this with the demands of this diverse and varied role.
The Service operates a 7 day service, currently covering the hours of 8am to 10pm, including weekends. You will be expected to work shifts to cover these hours and need to be flexible and rotating posts might be an expectation.

**Knowledge, Skills and Experience**:

- Good communication skills / the ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post
- Understanding of routine work procedures gained through a short induction period and on the job training
- Experience of working with members of the public or a desire to work in the Health Service
- Basic IT skills
