We are looking for a highly motivated, resilient, and flexible individual with a passion for improving the lives of young people, who understands neurodiversity as difference. The post requires developing key relationships with community providers, and match families and young people’s needs and interests to the right people and services, as well as support them to make this a success.

The team works across the local system with families, to ensure children and young people with the most complex needs, initially those at risk of admission or in hospital, receive appropriate personalised, integrated support. The post holder will work in partnership with case managers to find meaningful activities in the community for young people to engage and enjoy, so that they are able to stay (and thrive) in the community they know.

Whilst previous experience is desirable, we are looking for someone with the correct core skills for the role, and therefore a comprehensive induction and training will be provided.

Across Oxfordshire there is limited provision of intensive support for autistic and/or with Learning Disability CYP between CAMHS Tier 4 inpatient and universal CAMHS Tier 3 provision. To address this gap, Oxfordshire Clinical Commissioning Group (CCG) developed a business case to support CYP and families living in Oxfordshire with keyworker support and pilot this over 36 months.

These exciting pilot forms part of a national Keyworker Pilot developing new ways of working in partnership with NHS England, Oxfordshire Integrated care system and other agencies. Importantly the pilot is being driven by the lived experience of young people and their families, therefore Oxfordshire Parents Carer forums involvement are critical partners to shaping how we work in the future.

You will be an integral part of the multidisciplinary team, using your specialist skills to:

- provide a high quality specialist family therapy service to community service users/day patients/inpatients/carers/families who are referred into the Adult Community Eating Disorders Team, and Cotswold House (Oxford) inpatient eating disorder unit.
- offer systemic/family therapy advice, consultancy and supervision, and support the work of colleagues providing care and treatment, according to expertise.
- provide training to a range of qualified/unqualified staff across mental health areas
- partake in service transformation, through support/development of the new vision and development of treatment pathways

Since launch, the team has supported children and young people who are inpatients or at risk of being admitted to hospital. This same support will also be extended to the most vulnerable children with a learning disability and/or autism, including those who face multiple vulnerabilities such as looked after and adopted children, and children and young people in transition between services.

The service has been recognised for their innovation and outcomes and has been regularly invited to share the model and experience at a local and national level.

The post holder will be given full induction, including the ability to undertake keyworker specific and recovery star training, as well as join a wide community of practice network.

The service actively encourages professional development, and retain a strong ethos of supporting staff wellbeing.

**The key responsibilities within this role are**:

- Following the Case Managers assessment of individuals, to use social prescribing to help improve patient outcomes in a person-centred way, accessing the right help at the right time
- Supporting the Case Managers to follow up on Care, Education and Treatment Review (CETR) recommendations, or any other statutory frameworks
- Continuing to support young people through their involvement with inpatient services, should they become unwell in order to contribute to their timely discharge, well managed transitions and full implementation of CETR recommendations for community support
- To be responsible for keeping an up-to-date database of social prescribing opportunities in the Oxfordshire area
- To carry out other targeted interventions under supervision of the Case Managers to aid in the patient’s recovery, including exposure and scaffolding to enable access to services, promoting young people to develop their own solutions, led by their needs, wants and aspirations
