Job Title: Advanced Clinical Practitioner Overview Wednesdays, Thursdays and Fridays starting immediately - duty sessions 08:30-18:00 We are seeking a compassionate and skilled Advanced Clinical Practitioner (ACP) to join our dynamic and supportive healthcare team. Responsibilities Conduct advanced patient assessments, including history-taking, examination, diagnosis, and treatment planning, with care and sensitivity towards diverse communities. Provide clinical care as part of a multidisciplinary team, working collaboratively with physicians, nurses, and other health professionals to achieve optimal patient outcomes. Prescribe and review medications within scope of practice, ensuring adherence to local prescribing guidelines and clinical governance. Deliver personalized care plans, taking into consideration patients’ cultural, social, and emotional needs. Participate in clinical audits and quality improvement initiatives to ensure best practices are maintained. Serve as a mentor and educator to junior healthcare staff, students, and colleagues, contributing to the development of future clinical teams. Stay informed about advancements in clinical practices and contribute to implementing innovative care models. Identify and address healthcare disparities, ensuring equitable access to quality care for all patients. Qualifications Current licensure as a Registered Nurse (RN) with an accredited body and advanced certification as an Advanced Clinical Practitioner. Master’s degree in Advanced Clinical Practice or closely related field preferred. Proven clinical experience in acute, urgent, or primary care settings. Strong diagnostic and decision-making skills with the ability to manage complex patient presentations. Excellent communication and interpersonal skills to deliver empathetic and effective care to diverse patients from all walks of life. Commitment to diversity, equity, and inclusion and the ability to navigate culturally-competent practices in healthcare. Familiarity with electronic medical records and other healthcare technologies. Day-to-Day Conduct patient consultations, leveraging advanced clinical skills to diagnose and treat a wide variety of conditions. Collaborate with team members to manage patient caseloads effectively, ensuring continuity and coordination of care. Participate in team discussions and clinical handovers to ensure seamless care transitions. Engage in reflective practices and continuing professional development to enhance your clinical expertise.
