Oxford Health Foundation Trust is part of the Buckinghamshire, Oxfordshire and Berkshire integrated care system who are one of seven national accelerator sites for the NHS Urgent Community Response initiative. This position provides the successful clinician with an opportunity to work within our community services to support and deliver our Urgent Community Response.

We are delighted to offer the opportunity to undertake a role as a Specialist Practitioner ( band 7 clinician) in our community services. This is a full time post and will work across the North, Central or South Areas of Oxfordshire.

You will be embedded within the Community Teams which comprise of the Community Therapy Service, District Nursing Service, Community Matron service and Hospital at Home, however you will have the opportunity to work across many facets of Community services.

**If successful, you will be involved in the following**:
- As part of a multidisciplinary team the post holder will work autonomously to provide a high-quality service to patients face to face, particularly those requiring a urgent community response (2 hour response)
- You will be working as an Specialist Practitioner with a high degree of autonomy, the post holder will manage undifferentiated and undiagnosed conditions, using, developing and consolidating advanced clinical skills within own sphere of competence.
- The post holder will work collaboratively with the team and external providers, to ensure the right care, by the right person, at the right time and in the right place.
- Practitioners will aim to enable individuals to maintain or regain independence or ensure a plan is in place for them to achieve their optimum personal potential and to prevent unnecessary hospital admission
- They will support a learning environment that fosters a culture of clinical expertise and act as a role model and mentor to advanced skills learners from all community services and teams
- The service operates from 8am-8pm, 7 days a week, 365 days a year, and the post holder will work flexibly across shift patterns and occasional weekends to meet the needs to the services.
