You will also take responsibility for managing accurate and timely patient registrations. As a Care Coordinator, you will: Manage patient recalls for long-term condition reviews, ensuring patients attend regular check-ups and receive complete care from the relevant healthcare professionals.
Support patient registrations, ensuring records are accurate and up to date as part of the onboarding process. Work with patients, families, and carers to improve understanding of long-term conditions. Provide a point of contact for patients to help with queries, appointments, and access to high-quality information about their care options. Coordinate and navigate care across services, ensuring patients receive joined-up support from the right professional at the right time.
Collaborate with GPs and other professionals within the Primary Care Network (PCN) to proactively manage a caseload, including patients with long-term conditions, and refer them appropriately within the team.
