Optima is a CQC Registered service specialising in supporting Children, young people and adults with complex health needs across the Northwest of England. We are based in Denton ( 2 mins from the M60 junction)
Due to an internal re-organisation, we now have an exciting opportunity for an Experienced Care Co-ordinator.
Key Roles & Responsibilities:
- Day to Day responsibility & management of a team of approximately 20 Community Support Workers
- Caseload of approximately10 Service users and their families
- Working very closely with our ‘in-house’ Nursing Team to enable effective and timely training of your Team members in the clinical aspects of their role.
- First point of contact for all Service Users (their families) / Team members / External Professionals (Social Workers, Nurses etc) enquiries
- Assessment of Service Users needs at the beginning of their service and periodically throughout their support journey with Optima
- Management / Delivery of effective Support to your Team Members which includes reviewing Probationary periods and regular Supervisions with your Team.
- Training and developing your Team members in various topics such as Policy/Procedures/Personal Behaviour Expectations / Service Users ‘Awareness’ Sessions / Risk Assessments.
- Identifying Training Needs and Implementing Plans (4 to 8 weeks) to ensure Team members are trained and competent within agreed timescales
- Creating and Maintaining advanced Rotas which meet both Service Users needs and Team members work/life balance
- Interviewing & recruitment of new Team Members (in person and via ‘Microsoft Teams’)
- Collating service information for Senior Leadership Team and Commissioners
- Take part in regular paid ‘On Call’ support and advice to Team Members and Service users
About You:
- Experience in organising / managing home care / domiciliary services
- Ability to multi-task in a fast-paced environment
- Professional at all times & Calm under pressure
- Can use software programmes to input and extract data. Access Care Planner / People Planner an advantage
- Very confident in communication with people at various levels (written & verbal)
- Excellent understanding of the needs of people with additional and complex health needs
- Work as part of a small team but also work under own initiative when required
- Must be a driver (to attend meetings and undertake service user home visits across Merseyside Region) - Co-ordinator will be based in Head Office, Denton, Manchester
- Ideally have lots of life experience and bring ‘common sense’ thinking to problem solving daily.
- Always looks for a resolution & positives in life not only seeing the problems
- Thrive on making people’s lives better, with good organisation skills, delivering service user’s expectations and providing support to your team members
- Always looking for continuous improvement & adapting to change
- Excellent with Microsoft Outlook / Teams / Word and Excel (as Optima is a paperless and digital organisation)
- Confident and knowledgeable around Safeguarding Processes and management of ‘Fact Finding/Investigation’ process etc.
- Ability to ‘Grow’ with the business

Hours: Monday - Friday 9.00am to 5.00pm with On Call rota (shared with Team averaging 1 in 5 weeks)
