Job Purpose

As a Support Worker within the Day Service, you will provide support and care to individuals who use our service due to their learning disability, mental health and any associated physical disability or health need. You will support them to attend meaningful activities. You will work as an integral part of our team, providing excellent standards of support and care. Your work as a Support Worker may involve you working on a 1 to 1 basis with individuals that we support or as part of a bigger team providing support to individuals as required.

Main Duties: To provide care and support to Academy house service users in accordance with current best practice according to policy and procedures, agreed standards and legislative requirements, under the direction of the service manager and senior members of staff you will be a fully trained and competent member of academy house staff.

Person Centred Care and Supports
- You will help individuals to develop and maintain friendships and networks of support within the local community.
- You will be proactive in your approach to supporting service users maximise their potential, by enabling them to engage in meaningful activities and opportunities according to my personal plan.
- You will help individuals you are working with to maintain their personal safety, health and wellbeing, including supporting them with personal care. You will work to the personal plan and instructions agreed to achieve this.
- You will assist the individuals we support to participate in social, recreational and leisure activities that have been defined and chosen by them and you will work to my personal plan instructions agreed to achieve this.
- You will work in a key worker role to identified individual outcomes, taking the lead in ensuring their support is delivered in line with their Outcome Based Support Plans. You will be responsible for ensuring that these plans and any required risk assessments are reviewed and updated as required and in line with statutory and company expectations.
- You will contribute to the formation of Personal Plans and Outcome Based Support Planners for all individuals that we support.
- Encourage and promote independence for the service users to develop new skills and upkeep existing skills.

Planning for the Future

At Academy house we want everyone we work with to have a Personal Plan. This plan sets out ways for me to be supported in a way that I am comfortable with, setting out meaningful activities and how the service is meeting my outcomes and how my health needs are being met. The first step in completing my personal plan is usually to bring together everyone who is important in the individuals’ life to talk things over. As a Support Worker you may need to provide some practical help to the individual to make sure that these meetings happen and to help the person to express their own opinion about how they want their support to be.

You will also help the person to adapt and improve their personal Plan by assisting them at regular meetings and reviews.

Training and Supervision
- You will participate in a full Induction Training Programme.
- You will have the opportunity to participate in training relating to the specific support needs of the individual(s) that you are working with.
- You will participate in other training, as and when required.
- You will have the opportunity to be involved in Team and Individual Supervision and Appraisal and you will make yourself available for such sessions.
- You will participate in Team Planning and Team Building.
- You will take personal responsibility for putting your training and supervision into practice and for advising your Service Manager of any training needs.

Organisational Responsibilities
- You will adhere at all time to Academy house’s Policies and Procedures.
- You will maintain a safe working environment ensuring your own personal safety and the safety or others.
- You will ensure that work plans are adhered to and any deviation is discussed with your line manager/ Service manager at the earliest opportunity.
- To bring to the attention of the Senior support worker any amendments required to Rota.
- You will communicate clearly with colleagues so that our services operate effectively.
Personal Responsibilities
- You will encourage positive attitudes toward the individuals that we support.
- You will ensure that no action or omission on your part is detrimental to the interests, condition or safety of the individuals we support.
- You will work in a flexible way recognising the needs of the individuals we support.
- You will always represent the Company in a positive manner.
