The post holder is an experienced nurse, acting within their professional boundaries, providing care for the presenting patient, from initial history taking through to treatment. They will demonstrate safe, clinical decision making and expert care for the patients registered within the practice. They will work collaboratively with the multi-disciplinary team to meet the needs of the patients and support the delivery of policy and procedures. NMC requirements for advanced practice must be met.

To deliver a high standard of patient care as an Advance Nurse Practitioner in the community, using advanced autonomous clinical skills, and a broad in-depth theoretical knowledge base complementary to the doctor’s role

Provide holistic nursing service by assessment of physical, psychological and social needs including appropriate prescribing and referral
To facilitate and participate in multi-disciplinary education and clinical supervision

Participate in organisational decision making, interpret variations in outcomes and use data from information systems to improve practice

To play an active role in the development of practice development plans to the services.

COVID-19 Vaccination

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Ensuring organisational policies are understood and action taken to monitor staff adherence
- To participate in developing integrated care policies
- Ensuring that self and team members maintain accurate and contemporaneous clinical records in line with Trust policies and National standards, using current system provided
- To adhere to Risk management process in line with Trust Policy and to manage any incident in order to learn from it and minimise the risk of reoccurrence
- To maintain Clinical Governance Standards through audit, research and evidence gathering
- Remaining familiar with national, professional and local quality issues relevant to the delivery of care
- To be accountable for maintaining and monitoring measurable standards of care
- Ensuring that evidence is in place for Care Quality Commission compliance for the service
