Due to the expansion of our team, we have a fantastic job opportunity for an enthusiastic Support Coordinator like you, to join our Central Resource Team (CRT).
Here in Teesside, Durham and Sunderland, Home Group have a large number of support services, which help our customers who have individual and varying needs.
Instead of working in one service, our CRT gives you the opportunity to work across a wide range of supported services and alongside our specialised teams, helping to enrich our customers lives.
As our Support Coordinator, you’ll help change our customer’s lives for the better. You’ll undertake risk assessments, support planning, goal setting and regular reviews.
You’ll create individual and personalised support plans with and for customers. Each plan will look to meet the customer’s individual needs and aspirations whilst making sure they meet safeguarding and confidentiality requirements.
You’ll also have responsibility for coordinating our Support Workers and work collaboratively with external agencies, to assess and engage support for our customers.

**About you**
As our Support Coordinator, you’re passionate and motivated to help vulnerable people make positive changes to their lives. You’ll impress us with your passion to support others and can work on your own initiative.
You’ll promote independence and ensure the dignity, privacy and the rights of our customers by supporting them in a non-discriminatory person-centred way.
As our Support Coordinator, you’ll be flexible, resilient and adaptable in your approach as you’ll work wherever the business needs your support.
It would be amazing if you have had a job in this type of environment before, however it’s your “can do” positive approach and enthusiasm that are critical to making this role a success. We can nurture your talent, just let us know in your cover letter why you have a passion for supporting people.
To make sure that we keep our records straight, and customers safe, it's necessary that you have good IT skills or at the very least be enthusiastic to learn.
Finally, as you’ll travel around the area to work in our services, it is essential that you have access to a vehicle (we’ll pay your expenses to cover this use).
- When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one._

Our team is led by our amazing Senior Client Service Manager Yvonne who has worked for Home Group since 2014. Yvonne is perfectly placed to support you as you join. She started as a Support Worker and took full advantage of all the training and experience we offer here at Home Group.
Supporting Yvonne are our two wonderful Client Service Managers Benjiee and Shannon, who oversee the team on a daily basis. They started their Home Group journeys within the Central Resourcing Team and took the opportunities available to progress.
**Working hours**
You’ll work between the hours of 9:00am and 5:00pm, Monday to Friday.
We aim to be flexible in our approach, to meet the needs of our customer’s and ensure our colleagues have a good work life balance.
