This is a fantastic opportunity to work as part of innovative collaborative project. The Social Prescribing collaborative is a partnership between Action Together, Tameside Oldham & Glossop Mind, Age UK and Positive Steps. The role involves working with people referred into the programme to support them to reach their personal wellbeing goals by accessing support and opportunities in the voluntary and community sector.

About You:

- Working with people to provide support services to identify their strengths and to help them reach their aims
- Working within a partnership approach to develop successful working relationships with partners and other agencies
- Managing your own caseload and working independently
- Understanding of health and social care system/community-based health and wellbeing support services
- Working knowledge of emotional wellbeing and mental health issues
- Good working knowledge of safeguarding practices, information, data protection and professional boundaries
